An Ampullary adenoma is a benign glandular growth that develops at the ampulla of Vater, where the bile duct and pancreatic duct empty into the duodenum. Although benign, it is considered a precancerous lesion because some adenomas can progress to ampullary cancer over time.

Transduodenal ampullectomy (TDA) is a surgical procedure used to remove the ampulla and the adenoma through an incision in the duodenum while preserving the pancreas. It is less extensive than a Whipple procedure (pancreaticoduodenectomy) and may be considered when:
The lesion is an adenoma or very early-stage tumor.
Endoscopic removal is not feasible or has failed.
There is limited extension into the bile or pancreatic ducts.
The goal is to avoid the greater risks of a Whipple operation.
